Establishment Fee
The Establishment Fee can be changed to reflect any changes that may occur.
The changes should only be made if notification has been received from the National Dental Plan and/or Humm.
Any changes made that National Dental Plan and/or Humm have not authorised will be changed to the original setting at the application process.
Monthly Fee
The Monthly Fee can be changed to reflect any changes that may occur.
The changes should only be made if notification has been received from the National Dental Plan and/or Humm.
Any changes made that National Dental Plan and/or Humm have not authorised will be changed to the original setting at the application process.
NDP Payment Terms
The Payment terms can be changed to reflect any changes that may occur.
The changes should only be made if notification has been received from the National Dental Plan and/or Humm.
