eForms for Guest Users: Patient Experience

Listed below is how a patient can complete an eForm as a guest user. 

When the patient receives the SMS or email, they will click on the link within the message and this page will open. 

They will also receive another SMS or email containing a code

1. Upon receiving the SMS or email with link to eForm, patient will click link provided
   
2. To proceed and access the form, they must enter the confirmation code
   
3. The patient will be prompted to choose whether to receive the confirmation code via SMS or email
   
4. The patient can choose SMS
   
5. If the patient opts for email, they will need to enter their desired email address
   
6. Click Send Code
   
7. At Confirmation code enter the code
   
8. Click Apply (the patient can select to resend the code or change the method e.g. email or SMS)
   
   
9. A form will appear for the patient to complete
