eServices Reports from eServices Portal

These reports are run from within the eServices portal.
A link at the bottom is available for eServices related reports run from within Dental4Windows or Dental4Web. 

eServices D4W Connected Report

The eService D4W Connected report provides details of the eAppointments that have been created.

Run the Report
  1. Go to eService Portal> Menu> Analytical Dashboards > D4W Connected
  2. eService D4W Connected report
    1. Created: Select the period when the Appointment was created to base this report on 
    2. Appointment Date: Select the period the appointment is booked for to base this report on 
    3. Appointment Source: Select the origin of the booking to base this report on - this is beneficial for Patient Discovery users to determine which participating partner's website was used
    4. Status: Select the status to base this report on - the status refers to New Patient, Existing Patient, Pending and Undefined
    5. Click Retrieve
    6. Select .pdf or .csv: The report will be saved in this format

Report Details

Surgery: The practice name
Book Name: The name of the dentist's appointment book
Appointment Date/time: The date and time of the appointment
Patient Name & DOB: The name of the patient and their date of birth
Patient Contact Details: The patient's phone and email address
Appointment Source: Where the booking originated. This is beneficial for Patient Discovery users to determine which partner's website they’ve come from.
Rescheduled: It indicates that the appointment was successfully booked, but after it was confirmed, it was moved to a different time slot in the appointment book.
Status: The status of the patient e.g. New Patient New Patient: New Patient to the practice
      Existing Patient: Patient exists on the database
      Pending: This indicates a patient conflict. The conflict must be resolved in Dental4Windows/D4Web before the update appears here.
                      
If the conflict is resolved after the actual appointment (or first seen date), the status will change to New.
                      If resolved before the actual appointment (or first seen date), the status will update to New or Existing.
      Undefined: This status appears if there is an appointment conflict, or if the booking has been cancelled or rescheduled in Dental4Windows.
Created Date/Time: 
The date and time the appointment was created

eServices eForms Report

The eService eForms Report provides details of the eForms submitted at a single or multiple locations.

Run the Report
  1. Go to eService Portal Menu
  2. Select Analytical Dashboards > eForms Report
  3. eService eForms Analytical Report
    1. Submitted: Select the period when the reform was submitted to base this report on 
    2. Organization: This is optional, select the organization or all from the droplist
    3. Surgery: This is optional, select the surgery or all from the droplist   
    4. Click Retrieve 
    5. Select .xlsx. The report will be saved in this format
    6. Total Submitted: The total number of forms submitted. 
    7. Total Submitted by new patients: The total and percentage of the number of forms submitted by new patients

Surgery: 
The practice name 
Forms: Number of forms
Forms Online: Number of forms submitted online
Submitted by new patients: Number of forms submitted by new patients  
Submitted by all patients: Number of forms submitted by all patient
