Update / Create New Charting

It is advised that a patient’s chart be updated every check-up so that the chart is current and will reflect patient’s dental history more correctly.

Create New Chart

  1. Go to Patient Records > Charting (2D/3D) tab
  2. Charting menu > New Charting

A new date and time will be added with a blank chart in view.

Copy from Previous Chart

Next, the details from the previous chart must be copied over to the current one. 

  1. Still in Patient Records > Charting (2D/3D) tab
  2. Charting menu > Copy from a Previous Charting > Select appropriate option
    1. Current cond. to THIS current condition...
      1. Copies only the charting entered under Current Condition from another chart
    2. Completed Treat. required to THIS Current condition...
      1. Copies only the charting marked as completed from another chart, moving it to the Current Condition of this chart
    3. Incomplete Treat. required to THIS treat. required...
      1. Copies incomplete charting to this Treatment Required
    4. Both Current AND Completed Treat to THIS Current Condition...
      1. Copies both the completed treatment and current charting entries from another chart
    5. Current, Completed and Incomplete to THIS Chart...
      1. Copies all three at once, moving completed to current and incomplete to treatment required
      2. Select the appropriate chart to copy from
      3. Click OK

How to Chart Procedures

Within the Charting tabs of D4W, charting procedures allows a visual representation of the current state and planned treatment for the patient, whilst linking to a 3D Skull viewer and the Treatment Plan tab to help build comprehensive plans for the patient. 

2D Charting

  1. Go to Patient Records > Charting tab
  2. Select the surface(s) in either Current Condition or Treatment Required section
  3. Using the menus or icons, select the procedure to add to the surface(s)

  4. Shortcuts can be access from the Conditions menu   

3D Charting

  1. Go to Patient Records > 3D Charting tab
  2. Using the left mouse click, hold down to select the surface(s)
  3. Using the menus or icons, select the procedure to add to the surface(s)

  4. Teeth can be rotated as a whole using the control panel
  5. Individual teeth can be rotated by right clicking on the tooth > Select tooth
  6. Individual teeth can be presented onscreen by right click on the tooth > One Tooth Mode 

Charting Layers

The ability exists to indicate that on a particular tooth there are layers of treatment work.
For instance, through the crown there may be an Occlusal composite filling. You may add, delete and view different layers.

New Layer

  1. Go to Patient Records > Charting (2D/3D) tab
  2. For the first layer, proceed charting as per standard procedure
    1. Right click on the tooth and select New Layer

    2. An empty layer is created and is indicated with an icon. The new layer is the Upper Layer (or Layer 2)

    3. Right click on the tooth to toggle between the Upper Layer and Lower Layer

    4. A procedure or operation can now be bound to this layer

Delete Layer

Only the Upper Layer can be deleted.

  1. Go to Patient Records > Charting (2D/3D) tab
  2. Toggle to the Upper Layer
  3. Right click on the tooth and select Delete Layer

  4. Click Yes to confirm the deletion

Bind and Unbind an Item Number

In 2D Charting, when a root canal or implant item is charted, if the item is not bound to the layer before a new layer is created, the procedure will not be seen on the next layer. To ensure visibility we must bind the layer.

Bind Items

  1. Go to Patient Records > Charting tab
  2. Chart an RCT (for example)
  3. Create a new Layer
  4. Right click and select Bind Item "xxx" to layer x

  5. Switch to the Upper Layer or Lower Layer
  6. Chart an Implant (for example)   

  7. Right click and select Bind Item "xxx" to layer x
  8. When switching layers, the procedure will only be visible on the layer it has been bound to

Unbind Items

  1. Go to Patient Records > Charting tab
  2. Right click on procedure and select Unbind Item "xxx" from layer x

Mixed Dentition: Child to Child Mixed Chart or Adult Mixed Chart

Over time, selected (or all) teeth will need to move from Child to Adult teeth for correct charting. 

  1. Go to Patient Records > Charting (2D/3D) tab
  2. Charting menu > Show Mixed Chart
    1. No: An adult chart will show. This should be selected if patient has more adult teeth than deciduous





    2. Yes: A deciduous chart will show along with the molars of the Adult chart (mixed). This should be selected when this is a child as there will be more deciduous teeth 




  3. To change a tooth from Child to Adult or Adult to Child
    1. In Current Condition, select a surface (multiple teeth can be selected at once) > Right mouse click > Toggle adult/deciduous


Delete Multiple Objects in Charting

When more than one object is present on a tooth/surface, and/or multiple teeth are selected, a confirmation message of what to remove will appear.

  1. From the Patient Details > Charting 2D / Charting 3D tab
  2. Select the objects > Click Delete

    1. Select the objects to remove
    2. Click Delete

View Document Manager in 2D or 3D Charting

If Document Manager is turned on in Charting it appears on the right side of the screen. Files can be accessed from the Charting and 3D Charting tab. 

  1. Go to Patient Records > Charting tab
  2. File/View menu > Show Images-Video-Document Mgr

  3. To have Document Manager visible in 3D Charting tab
    1. Go to Patient Records > 3D Charting tab
    2. Charting menu > Set Display Options...
    3. In Show/Hide section tick Show Document Manager > OK
