Medical History Forms in D4Web

Once a Patient has had a letter created from a template that is marked as a "Medical history Form", the user will be able to open that "Medical history Form" from the Patient Appointment slot. 

Create the Medical History Form

Before marking a template as a "Medical history Form", the template itself must exist the Letters pages.

Mark a template as a "Medical history Form"

  1. Click the navigation menu > Patients > Letters page
  2. Select the category and highlight the template name
  3. Template toolbar > Mark as Medical History Form

  4. The marked template now has a blue envelope as its marker

Open the Medical History form for specific patient

  1. Find the patients appointment
  2. Right click > Print > Medical History Form

  3. The template will come up
  4. Make changes / Add information to the Letter for the selected patient as needed
