Medical History Forms in D4Web

Once a Patient has had a letter created from a template that is marked as a "Medical history Form", the user will be able to open that "Medical history Form" from the Patient Appointment slot.
Create the Medical History Form
Before marking a template as a "Medical history Form", the template itself must exist the Letters pages.
Click here for instructions on how to create a New Letter TemplateMark a template as a "Medical history Form"
- Click the navigation menu > Patients > Letters page
- Select the category and highlight the template name
- Template toolbar > Mark as Medical History Form

- The marked template now has a blue envelope as its marker

Open the Medical History form for specific patient
- Find the patients appointment
- Right click > Print > Medical History Form

- The template will come up
- Make changes / Add information to the Letter for the selected patient as needed
