Where to find the Charting pages
Click the navigation menu > Patients > Charting or 3DCharting page
Note: Where a feature is 2D or 3D specific, the instruction will be noted as such
Create a New Charting Record
It is advised that a patient’s chart be updated every check-up so that the chart is current and will reflect patient’s dental history more correctly.
- Charting toolbar > New Charting
A new date and time will be added with a blank chart in view

Copy From a Previous Charting
Once a new chart has been created, the details from the previous chart should be copied over to the current one to avoid re entering.
Select how much information you wish to bring forward from the list of options available (for full details to be copied, we recommend option e)
- Charting toolbar > Copy from a Previous Charting
- Select:
- Current cond. to THIS current condition...
Copies only the charting entered under Current Condition from another chart - Completed Treat. required to THIS Current condition...
Copies only the charting marked as completed from another chart, moving it to the Current Condition of this chart - Incomplete Treat. required to THIS treat. required...
Copies incomplete charting to this Treatment Required - Both Current AND Completed Treat to THIS Current Condition...
Copies both the completed treatment and current charting entries from another chart - Current, Completed and Incomplete to THIS Chart...
Copies all three at once, moving completed to current and incomplete to treatment required

- Next, select the appropriate chart (date) to copy from
- Click OK

Charting CDBS Procedures
When charting a CDBS patient, use the Child Benefit Scheme menu to add CDBS items onto the Treatment Plan.
If the standard menus and/or icons are used, the standard ADA items will appear instead on the Treatment Plan.
If not already activated, click here to activate CDBS in D4Web.- Select the tooth surface in Treatment Required
- Operations toolbar > Child Benefit > Select the appropriate procedure

When more than one object is present on a tooth/surface, and/or multiple teeth are selected, a confirmation message of what to remove will appear.
- Select the Teeth / Surfaces to be deleted
- Click Delete icon

- Only highlight the rows to be deleted
- Click DELETE

Charting Layers
The ability exists to indicate that on a particular tooth there are layers of treatment work.
For instance, beneath a crown there may be a filling or a pin / post.
You may add, delete and view different layers.
- For the first layer, proceed as per standard Procedure (e.g. chart a MO composite)
- Right click on the tooth and select New Layer
D4Web places an empty layer that you may chart on

- Add the next procedure required. i.e.: Crown
Above / Below the selected tooth indicates which layer you are currently on

- To view each layer:
- Right click on selected tooth to view another layer
Select Upper layer or Lower layer

- To delete a layer:
- Right click on selected tooth
- Select Delete layer
- Check details
- Click YES

Binding Item Numbers (2D only)
In 2D Charting, when a root canal or implant item is charted, if the item is not bound to the layer before a new layer is created, the procedure will be seen on every layer.
To ensure visibility in the correct layer we must bind the RCT or Implant to a layer.
- Chart an RCT
- Right click and select > Bind Item "417" to layer 1

To unbind Items, Right click on procedure and select > Unbind Item "XXX" from layer X
Mixed Dentition
Toggle Adult Chart with Deciduous Teeth
It is possible to change a tooth from adult to child or vice versa. This is commonly used for retained child/deciduous teeth.
- Charting toolbar > Show Mixed Chart
- Click NO (An adult chart will show with the ability to toggle from adult to child)

- To change a tooth from adult to deciduous:
- In Current Condition, select a surface (multiple teeth can be selected at once).
- In Charting: Right click > Toggle the teeth view: adult - deciduous

- In 3D Charting: Right mouse click > Toggle adult/deciduous

Toggle Deciduous teeth to Mixed or Adult Chart
Over time, selected (or all) teeth will need to move from Child to Adult teeth for correct charting.
- Charting toolbar > Show Mixed Chart
- Click NO: An adult chart will show. This should be selected if patient has more adult teeth than deciduous.
- Click YES: A chart will show combination of CHILD & ADULT with the ability to toggle from adult to child

- In Current Condition section, select a surface (multiple teeth can be selected at once).
- In Charting: Right click > Toggle the teeth view: adult - deciduous

- In 3D Charting: Right click > Toggle adult/deciduous

Toggle Mixed Chart to Adult Teeth
To change a tooth from child to adult when in a mixed chart.
- Charting toolbar > Show Mixed Chart
- Click YES: A chart will show combination of CHILD & ADULT with the ability to toggle from adult to child

- In Current Condition, select a surface (multiple teeth can be selected at once).
- In Charting: Right click > Toggle the teeth view: adult - deciduous

- In 3D Charting: Right click > Toggle adult/deciduous
