Clinical Notes Log Audit
All notes in D4Web can be audited. The audit will show the original state and current state of the notes, as well as who created and/or modified the notes.
Users must be logged in as themselves to correctly enter and track notes. If a login is shared, the notes will sit against that login name, legally.

- Date: Select the period to base this report on
- Location: Select the location or <Any Location > if applicable
- Providers: Select the Provider or <Any Provider>
- Show inactive providers and staff as well: Tick to include inactive Providers
- Show records produced by an unknown user: Tick to include records which do not indicate who completed the action due to no security being set
- Include log audit for removed items: Tick to include details of items deleted
- Include log audit for removed invoices: Tick to include details of reversed invoices
- Click OK
Report Details
- Patient The Card Number and Patient Name
- Item The item the notes are attached to
- Date The date the notes were added
- Provider The provider of the item the notes are attached to
- Created By The name of the D4Web User that wrote the notes.
- If Unknown is shown, there is no security on writing clinical notes and therefore cannot be adequately tracked.
- Updated By If the original note has been updated, the details will be shown.
Clinical Treatment Notes by Item
This report is used to view all clinical notes information relating to a specific item(s).
- Double click to open the Clinical Treatment Notes by Item report
- Report parameters:
- Date: Select the period to base this report on
- Location: Select the location to base this report on
- Provider: Select the Provider or <Any Provider>
- Procedures: Select the items to base the report on
- Show obsolete items as well: Tick to include items marked as obsolete since use
- Show inactive providers as well: Tick to include inactive Providers
- Click OK

Report Details
Within the report, it is possible for those with security permission to edit/add to the notes.
- For Provider: The Provider of the item which notes are attached to
- Item: The item the notes are attached to
- Patient: The Patient the item and notes are attached to
- Card ID: The Patients' card number
- Phone_Home: The Patients' home phone number
- Phone_Work: The Patients' work phone number
- Treated Date: The date of the item
- Notes: The notes attached to the item
- The notes can be edited provided the user has security permission to do so
The No Quote/Planned Treatment Performed report will show all patients that had items entered into Treatment that were not transferred from a Treatment Plan.

- Date: Select the period to base this report on
- Location: Select the location or <Any Location > if applicable
- Providers: Select the Provider(s) to include
- Show inactive providers as well: Tick to include inactive Providers
- Click OK

Report Details
- Date: The date of the item
- Patient Name: The patient the item is attached to
- Patient ID: The Card Number of the patient
- Treatment Items: The item attached to the patient which should have been Treatment Planned
Prescription by Provider/Medication

Prescriptions issued within a period of time, with ability to group report by Providers or by medications prescribed.

- Report Parameters:
- Date range: Select the date range to base this report on. Date range refers to patients' prescription creation
- Location: Select the location or <All> to base this report on, if applicable
- Group By Providers: Select radio button if the report should be viewed by provider groups
- Group by Medications: Select radio button if the report should be viewed by medication groups
- Provider: Select the main Provider(s) or <All> of the patients to include in the report
- Medications: Select the medication(s) or <All> of the medications to include in the report
- Show inactive providers as well: Tick to include inactive Providers
- Click OK

Report Details
Provider: The Main Provider of the listed patients
Prescriber No: (AUS) The prescriber number allocated to the provider
DC Registration Number: (NZ) The NZ Dental Council Registration number
Date Created: the date the prescription was created
Patient Name: Includes Card number
Medication: The medication name and details
Treatment Incomplete Report
The Treatment Incomplete report provides a list of patients with incomplete treatment and no future appointments.
The patients have to have treatment done previously (treatment items entered in Treatment page), and fit in one of the situations:
- Treatment is completed box un-ticked in patient's file
- Treatment is completed box ticked, but patient still has an active treatment plan
